    • Exploration of the role of PIVAS pharmacists in optimization of parenteral nutrition prescription systems and medication safety monitoring

    • Our PIVAS pharmacists have led the improvement of the audit rules for parenteral nutrition orders and the display of information on parenteral nutrition infusion labels in the PIVAS system. We have strengthened the process management of parenteral nutrition infusion preparation, formulated detailed quality control inspection rules for the preparation process, and created the "Clinical Safety Monitoring Form for Extraintestinal Nutrition Infusion Products" to monitor and inspect any abnormalities in finished infusion products, infusion operations, and complications that occur during the use of parenteral nutrition infusion products. By comparing and optimizing the efficiency of parenteral nutrition order review, unreasonable order rate, vascular access selection and infusion speed standardization rate, mixing error rate, and abnormal situation data in clinical use before and after the inspection, the implementation effect of the above optimized inspection measures is examined. The optimized prescription review system achieved automatic review of parenteral nutrition orders, improving the efficiency of order review. The average time for reviewing one parenteral nutrition order was reduced from about 1 minute to 10 seconds. The unreasonable rate of parenteral nutrition orders decreased by 31.87% compared to before optimization; The error rate of parenteral nutrition allocation has decreased by 56.55% compared to before the inspection; The standardization rate of vascular pathway selection and infusion speed has increased by 13.29% and 3.54% respectively compared to before label information optimization. PIVAS pharmacists found 5 cases of abnormal infusion procedures among 298 patients who were randomly selected to use parenteral nutrition solution, and promptly intervened.
