      摘要:OBJECTIVE:To investigate the accessibility of antibiotics in essential medicine list in medical institutions of Nanjing area ,and to provide reference for formulating and improving related drug policy. METHODS :Taking the original and imitated drugs as the objects of investigation ,the standard investigation method jointly formulated by WHO/HAI was adopted to select 35 kinds of antibiotics which were included in Essential Medicine List (2018 edition)and International Drug Price Indicator Guide(2015 edition),and to investigate 3 dimenions such as the availability (proportion of institutions that could provide this drug),price(calculated by MPR )and affordability (the ratio of drug expenditure to the daily minimum wage of non-technical government workers )of them in 13 second-class or above medical institutions in Nanjing area by online questionnaires during Jan.-Jun. 2019,so as to put forward the suggestions for drug policy formulation. RESULTS :The median availability of original drugs was 0,with a range of 0 to 100%;the median availability of generic drugs was 30.80%,with a range of 0 to 100%. The median MPR of original drugs was 5.54,with a range of 1.96 to 18.83;the median MPR of generic drugs was 1.76,with a range of 0.16 to 7.66. Median affordability of original drugs was 8.68,with a range of 1.19 to 41.79;the median affordability of generic drugs was 0.52, with a range of 0.03 to 16.80. CONCLUSIONS:Generic drugs are more available than the originals. The price of original drugs is generally very high , while the price of generic drugs is mostly cheaper. The affordability of original drugs is really poor , and the 规。E-mail:shaorong118@163.com affordability of generic drugs is totally good. There are stillrooms for further improvement in the overall accessibility of essential medicines. At present ,ensuring the accessibility of original drugs may be more helpful to meet the needs of treatment. It is suggested to update and adjust the essential medicine list based on the clinical medication requirement ,popularize the knowledge about essential medicines ,properly adjust the price of original drugs and ease the treatment burden of patient. It is supposed to take into full consideration about the regional factors and the needs of different medical institutions when making drug policy.

      摘要:OBJECTIVE:To st udy preventive effect and mec hanism of ginsenoside Rg 1 on focal cerebral ischemia-reperfusion injury(CIRI)model rats. METHODS :Totally 78 SD rats were randomly divided into sham operation group ,model group , butylphthalide control group (positive control ,10 mL/kg),ginsenoside Rg 1 low-dose,medium-dose and high-dose groups (10, 20,40 mg/kg),with 13 rats in each group. Administration groups were give relevant medicine intraperitoneally ,sham operation group and model group were given constant volume of normal saline intraperitoneally ,once a day ,for consecutive 7 d. After medication,except for the sham operation group ,focal CIRI model was induced by middle cerebral artery occlusion (MCAO) method in other groups. After modeling ,neurological deficit scoring was performed according to the modified neurological difict scoring standard ; TTC staining was used to detected the percentage of cerebral infarction of rats ;the cerebral water content was measured by dry/wet weight method ;serum contents of IL- 1β and IL-6 were detected by ELISA ;the protein expressions of p-p 38 MAPK and p-NF-κB p65 in cerebral tissue were determined by immunohistochemistry and Western blotting assay. RESULTS : Compared with sham operation ,neurological deficits score ,percentage of cerebral infarction and cerebral water content ,serum contents of IL- 1β and IL-6,positive expression numbers of cells and protein expressions of p-p 38 MAPK and p-NF-κB p65 in cerebral tissue were increased significantly in model group (P<0.05 or P<0.01). Compared with model group ,above index levels of administration groups were all decreased significantly (P<0.05 or P<0.01),and the effect of ginsenoside Rg 1 had a dose-dependent trend ;there was no significant difference of all above indexes between ginsenoside Rg 1 middle-dose,high-dose groups and butylphthalide control group (P>0.05). CONCLUSIONS :Ginsenoside Rg 1 has a certain preventive effect on focal CIRI model rats ,the mechanism of which may be associated with down-regulating the protein expression of p-p 38 MAPK and p-NF-κB p65,inhibiting the release of inflammatory factors such as IL- 1β and IL-6.

      摘要:OBJECTIVE:To investigate the anti- hepatic fibrosis (HF)effects of Qiwei qinggan powder and explore its possible mechanism. METHODS :Male Wistar rats were randomly divided into blank group ,HF model group ,Qiwei qinggan powder low-dose,medium-dose and high-dose groups [ 135,270,405 mg/(kg·d),by total amount of crude drugs] ,with 12 rats in each group. Except for blank group ,other groups were given 50% CCl4-peanut oil solution intragastrically (2 mL/kg,twice a week ,for consecutive 8 weeks) to induce HF model. At same time , blank group and model group were given constant volume of 0.5% CMC-Na solution intragastrically ;administration groups were given relevant medicine intragastrically ,once a day ,for consecutive 8 weeks. General situation of rats were observedand liver morphology was observed after last administration and hepatic indexes were detected. The contents of liverfunction indexes (ALT,AST,ALP,HYP)in serum and the expression of α-SMA in hepatic tissue were determined , and HE and Masson staining were performed to observe the histopathology. Using the difference multiple of expression quantity as the index ,TMT technology was used to screen the differentially expressed protein in medicine group (combining the liver tissue samples of Qiwei qinggan powder groups )and HF model group. Uniprot-GOA database and KAAS ,KEGG mapper online tools were used to analyze GO and KEGG pathway enrichment. RESULTS :The rats in the blank group were in good health ;the liver was bright red and smooth ,the liver lobules were intact ,no degeneration and necrosis ,inflammatory cell infiltration or fibrous tissue proliferation was found. Compared with blank group ,the rats in HF model group had poor diet ,depressed spirit ,disordered and lusterless fur ;the liver was dark red or yellow with rough surface ,hard texture ,inflammatory cell infiltration ,fiber tissue destruction ,bridge connection and so on ;the hepatic index ,the contents of liver function indexes and the expression of α-SMA were increased significantly (P<0.05). Compared with HF model group ,above symptoms of rats were improved to different extent in different dose groups of Qiwei qinggan powder ;hepatic index in Qiwei qinggan powder low-dose group ,the content of ALP in high-dose group ,the contents of ALT,AST and HYP and the expression of α-SMA in different dose groups were decreased significantly (P<0.05). A total of 42 differentially expressed proteins related to HF were screened ,of which 15 were up-regulated and 27 were down-regulated in expression,including fatty acid binding protein 4(FABP4),cholesterol 7α-hydroxylase(CYP7A1). The results of enrichment analysis showed that the differentially expressed proteins were mainly enriched in extracellular space ,blood particles and other cell parts,involving the molecular functions of oxidoreductase activity and fatty acid binding ,the biological processes of the regulation of heterotypic cell adhesion ,protein activation cascade ,as well as retinol metabolism ,arachidonic acid metabolism ,PPAR and other signal pathway. CONCLUSIONS :Qiwei qinggan powder can reduce the hepatic index ,ALT,AST,ALP and HYP contents in serum ,down-regulate the expression of α-SMA,improve the degree of inflammation and fibrosis of liver tissue ,and have a certain protective effect on rats. The anti-HF mechanism of it involves multiple targets and signal pathways ,such as FABP 4, CYP7A1 and PPAR.

      摘要:OBJECTIVE:To study the protective effects of Lespedeza cunea ta extract on glutamate-induced hippocampal cells HT22 injury of mice and its possible mechanism based on Nrf 2/HO-1 signaling pathway. METHODS :Using glutamate (5 mmol/L) to extablish the injury model of HT 22 cells. Using water soluble vitamin E as positive control (50 µmol/L),MTT assay was used to detect the effects of 0(blank control ),25,50,100 µg/mL petroleum ether extract ,dichloromethane extract ,ethyl acetate extract of L. cuneata on the proliferation of glutamate-induced injury cellsafter pretreated for 12 h. Using water soluble vitamin E as positive control (50 µmol/L),DCFH-DA assay was used to detect the effects of 0(blank control ),25,50,100 µg/mL L. cuneata dichloromethane extract on the level of active oxygen (ROS)in glutamate-induced injury cells after pretreated with 12 h. Using HO-1 agonist CoPP as positive control ,Western blotting method was used to detect the effects of 0(blank control ),25,50,100 µg/mL L. cuneata dichloromethane extract on the protein expression of HO- 1 after treated for 24 h. Western blotting method (treated for 0.5,1,1.5 h)and immunofluorescence staining (treated for 1 h)were used to detect the effects of 100 µg/mL L. cuneata dichloromethane extract on protein expression of Nrf 2 inside and outside the nucleus. After HO-1 gene was silenced by small interfering RNA (Si RNA )transfection technology ,the effects of 100 µg/mL L. cuneata dichloromethane extract on the survival rates of glutamate-induced injury cells and the level of ROS were detected. RESULTS :Compared with blank control ,50, 100 µg/mL L. cuneata dichloromethane extract could significantly improve the survival rate of glutamate-induced injury cells (P< 0.05),while reduced the level of ROS (P<0.05). 25,50, 100 µg/mL L. cuneata dichloromethane extract could increase the protein expression of HO- 1 in cells(P<0.05),while 100 com µg/mL L. cuneata dichloromethane extract could significantly decrease the protein le vel of Nrf 2 in cytoplasm and increasethat in nucleus (P<0.05). After HO-1 gene silencing ,the effects of L. cuneata dichloromethane extract on the proliferation promotion of glutamate-induced injury cells and the reduction of ROS level were reversed (P<0.05). CONCLUSIONS :L. cuneata dichloromethane extract can protect HT 22 cells against injury induced by glutamate through activating Nrf 2 pathway,inducing HO- 1 expression.

      摘要:OBJECTIVE:To stud y the effects of Zhitong shunqi capsule on JAK/STAT signaling pathway of chronic atrophic gastritis(CAG)model rats ,and to provide reference for clarifying its mechanism of improving CAG. METHODS :Totally 60 SD rats were randomly divided into blank group ,model group ,positive control group [vitacoenzyme ,0.09 g/(kg·d)],Zhitong shunqi capsule low-dose ,medium-dose and high-dose groups [ 0.75,1.5,3 g/(kg·d)],with 10 rats in each group. Except for blank group,other groups were given N-methyl-N′-nitro-N-nitrosoguanidine freely drinking combined with abnormal ingestion method to induce CAG model. After end of modeling ,administration groups were given relevant medicine intragastrically ,blank group and model group were given constant volume of water intragastrically ,for consecutive 28 d. After end of medication ,ELISA method was used to determine the serum levels of IL- 1β and IL-6;the gastric mucosa tissue pathologic change was observed by HE staining;mRNA and protein expressions of JAK 1,STAT3,SOCS-3 and c-Myc in gastric mucosa tissue were detected by real-time PCR and Western blotting assay. RESULTS :Compared with blank group ,the sparse and irregular glands with deep staining cell nucleus could be seen in the gastric mucosa of rats in model group ;serum levels of IL- 1 β and IL-6,mRNA and protein expressions of JAK 1,STAT3 and c-Myc in gastric mucosa tissue were increased significantly (P<0.05),while mRNA and protein expression of SOCS- 3 in gastric mucosa tissue were decreased significantly (P<0.05). Compared with model group , glandular arrangement of gastric mucosa was more orderly and the number of heavy stained cells was less in administration groups;serum level of IL- 6 and mRNA expression of c-Myc in gastric mucosa of rats was decreased significantly in Zhitong shunqi capsule low-dose group (P<0.05),while the protein expression of SOCS- 3 was increased significantly (P<0.05); serum levels of IL- 1β and IL-6,mRNA expressions of JAK 1,STAT3 and c-Myc in gastric mucosa tissue ,protein expression of JAK1 were decreased significantly in Zhitong shunqi capsule medium-dose group (P<0.05),while mRNA and protein expression of SOCS- 3 was increased significantly in gastric mucosa tissue (P<0.05);above indexes were improved significantly in positive control group and Zhitong shunqi capsule high-dose group (P<0.05). Compared with positive control group ,mRNA and protein expression of STAT 3 in gastric mucosa tissue were decreased significantly in Zhitong shunqi capsule high-dose group (P<0.05). CONCLUSIONS:Zhitong shunqi capsule can improve CAG model rat to certain extent ,the mechanism of which may be associated with the down-regulation of mRNA and protein of JAK 1,STAT3 and c-Myc ,and up-regulation of mRNA and protein of SOCS-3.

      摘要:OBJECTIVE:To simultaneo usly determine the contents of atractylenolide Ⅱ ,β-eudesmol,atractyloxin and atractylone in Atractylodes chinensis ,and to evaluate the quality of A. chinensis with different growth years combined with color difference principle. METHODS :HPLC method was adopted. The determination performed on Agilent Eclipse XDB-C 18 column with mobile phase consisted of acetonitrile- 0.2% phosphoric acid (gradient elution )at the flow rate of 1.0 mL/min;the detection wavelengths were set as 208 nm(atractylenolide Ⅱ,β-eudesmol),340 nm(atractyloxin)and 220 nm(atractylone);the sample size was 15 μ L. Using atractyloxin as reference,QAMS was adopted to establish relative correction factors (RCFs) of atractylenolideⅡ,β-eudesmol and atractylone ;the content of each component in A. chinensis with different growth years were calculated. The contents of above 4 components were determined by external standard method and then compared with the results of QAMS. The color difference values of A. chinensis powder were measured based on color difference principle. The correlation analysis of above 4 components content with color was carried out by Pearson correlation analysis. RESULTS :The separation degree of atractylenolide Ⅱ,β-eudesmol,atractyloxin and atractylone in A. chinensis was higher than 1.5. The linear range were 1.01-10.10,3.30-33.00,4.40-44.00,5.34-53.40 μg/mL,respectively. RSDs of precision ,reproducibility and stability tests were all lower than 2%,while the average recovery rates were 101.34%-104.67%(RSD<1.5%,n=6). Using atractyloxin as reference , RCFs of atractylenolide Ⅱ,β-eudesmol and atractylone were 3.896 7,5.928 2,9.727 9,with RSD of 0.35%,2.89%,0.36% (n=6),respectively. Relative deviation of 3 components (except for atractyloxin ) in 24 batches of A. chinensis ranged 0.03%-1.45% between QAMS and external standard method ,which indicated that the results of two methods were consistent ,and the content of each component increased with the increase of growth years. Atractylenolide Ⅱ,β-eudesmol,atractyloxin and atractylone in A. chinensis had significant negative correlation with its color shade (L*),total color difference (E*ab)(P<0.01), and significant positive correlation with color red-green direction (a*), color yellow-blue direction (b*)(P<0.01). CONCLUSIONS:The established HPLC-QAMS method can be used for the determination of atractylenolide Ⅱ,β-eudesmol, atractyloxin and atractylone in A. chinensis . The longer the growth period is ,the higher each component content is. The color of A. chinensis is closely related to the content of each component ,and the content of effective components is higher in A. chinensis with dark yellowish brown color.

      摘要:OBJECTIVE:To investigate the effects of Guizhi ful ing capsules and its principal components (paeoniflorin, paeonol and amygdalin )on the intestinal flora of primary dysmenorrhea model rats. METHODS :Female SD rats were randomly divided into normal group ,model group ,capsule group(Guizhi fuling capsule ,1 000 mg/kg),paeoniflorin group (15.0 mg/kg), paeonol group (10.3 mg/kg)and amygdalin group (12.1 mg/kg),with 6 rats in each group. Except for normal group ,other groups were given estradiol benzoate subcutaneously on the back of rats and oxytocin intraperitoneally to induce primary dysmenorrhea model. From the 4th day after subcutaneous injection of estradiol benzoate ,normal group was given constant volume of normal saline intragastrically ;model group was given constant volume of 0.5%CMC-Na solution intragastrically ;administration groups were given relevant medicine intragastrically ,once a day ,for consecutive 7 days. The writhing times and the contents of and MDA in uterus tissue of rats were determined ,and then com the contents of short-chain fatty acids (SCFAs)such as acetic acid,propionic acid ,butyric acid in colonic contents were detected by GC method. Using diver sity index as index , Rep-PCR and Eric-PCR were used to evaluate the d iversity of intestinal flora in feces of rats. RESULTS :Compared with normal group,the writhing times of rats were increased significantly in model group ;the contents of NO and MDA in uterus were increased significantly ,while the contents of acetic acid ,propionic acid and butyric acid in colonic contents and total content of SCFAs were decreased significantly (P<0.05 or P<0.01);the number of DNA electrophoresis bands of intestinal flora was significantly reduced ,the brightness of most bands was significantly reduced ,and the diversity indexes (by Rep-PCR and Eric-PCR method ,hereinafter)1 h after administration were significantly reduced (P<0.05 or P<0.01). Compared with model group,writhing times of rats were decreased significantly in capsule group ,paeoniflorin group and paeonol group ;the contents of NO in uterus of rats in capsule group and paeoniflorin group as well as the contents of MDA in capsule group ,paeoniflorin group and paeonol group were decreased significantly (P<0.05 or P<0.01);the propionic acid content and total content of SCFAs in colon of rats in capsule group ,the contents of acetic acid ,propionic acid and butyric acid ,total content of SCFAs in paeoniflorin group as well as the contents of propionic acid and butyric acid ,total content of SCFAs in paeonol group were increased significantly;the content of isovaleric acid was decreased significantly in paeoniflorin group (P<0.05 or P<0.01);DNA electrophoresis bands and its brightness of intestinal flora changed to different extents in administration groups ,and the diversity indexes of intestinal flora 1 h after administration were increased significantly in capsule group and paeoniflorin group ,while those indexes were decreased significantly in paeonol group and amygdalin group (P<0.05 or P<0.01). CONCLUSIONS :Guizhi fuling capsules can significantly reduce writhing times and the contents of NO and MDA in uterus of primary dysmenorrhea model rats. At the same time ,the capsules also can regulate SCFAs content in colonic contents and intestinal flora diversity of rats. The above effects may be related to paeoniflorin and paeonol in the capsules.

      摘要:OBJECTIVE:To investigate the effects of ligustrazine on ge ne expression of acute spinal cord injury (SCI)model rats. METHODS :Male SD rats were randomly divided into sham operation group (group A ,6 rats),model group (group B ,6 rats at each time point ,12 rats in total )and ligustrazine intervention group (group C ,6 rats at each time point ,12 rats in total ). Acute SCI model was established by modified Allen ’s method in group B and C. After modeling ,group C was given ligustrazine 100 mg/kg intraperitoneally ,while group A and B were given constant volume of normal saline intraperitoneally ,once a day ,for consecutive 7 d or 14 d(i.e. group B 7d and B 14d,group C 7d and C 14d). BBB scoring was conducted in each group before modeling , 7 and 14 days after modeling. HE and Nissl staining observation were also carried out for spinal cord specimen. The differentially expressed genes (DEGs)between group A and group B ,group B and group C were analyzed by transcriptome sequencing. The enrichment of Gene Ontology (GO)and KEGG signaling pathway was analyzed by GO database and KEGG database. RESULTS : Compared with group A ,BBB scores of group B were decreased significantly 7 d and 14 d after modeling (P<0.05),and the number of nerve cells and Nissl body in spinal cord tissue were decreased. Compared with group B ,BBB scores of group C were increased significantly at above time points (P<0.05),and the number of nerve cells and Nissl body in spinal cord tissue were increased. The numbers of DEGs of group A and group B 7 d, group A and group B 14d,group B 7d and group C 7d,group B 14d FAA380076) and group C 14 d were 886,1 404,70,66,respectively. The genes with opposite expression trend included Ncmap,Prx, Gabrq, Gabrg2, etc. The enrichment cell component , molecular function ,biological process of DEGs were different 630179114@qq.com in each group ,mainly involving lyocytosis ,lysosome,plasmamembrane,homotype protein binding ,immune response ,ion channel activity ,immune response (group A and B );basolateral plasma membrane ,exodeoxyribonuclease activity ,response to INF-γ (group B 7 d and C 7 d);extracellular domain ,receptor regulatory activity ,phenolic compound metabolism process (group B 14 d and C 14 d). DEGs enriched in cytokine-cytokine receptor interaction(group A and B );CAMs,complement and coagulation cascades and Hedgehog signaling pathway (group B 7d and C 7d); retrograde endocannabinoid signaling ,neuroactive ligand-receptor interaction ,PPAR signaling pathway ,GABA ergic synapse (group B 14 d and C 14 d),etc. CONCLUSIONS :Protective effect of ligustrazine on acute SCI model rats may be associated with inflammatory response ,immune response/regulation ,neuron ion channel ,cytokine-cytokine receptor interaction ,neuroactive ligand-receptor interaction and regulation of GABA ergic synapse activity .

      摘要:OBJECTIVE:To study the prepar ation technology of gastric floating tablets of Schisandra chinensis total lignans (SCTL),and evaluate the quality of prepared tablets. METHODS :Based on single factor test ,the orthogonal experiment was conducted to optimize the formulation of SCTL gastric floating tablets with the contents of hydroxypropylmethylcellulose (HPMC) K15M,NaHCO3 and microcrystalline cellulose as the factors ,using starting time ,holding time and cumulative release rate of gastric floating tablets as evaluation indexes. The properties ,weight difference ,floatability and accumulative release rate of the prepared SCTL gastric floating tablets were determined. The gastric floating tablets were qualitatively identified by TLC ,and the contents of schisandrin A and total lignans were determined by HPLC and UV spectrophotometry. RESULTS :The optimal formulation of SCTL gastric floating tablets was made up of 23% SCTL extract ,20% HPMC K 15M,40% microcrystalline cellulose,15% sodium bicarbonate ,1% octadecyl alcohol and 1% polyvinylpyrrolidone. The results of detection of this preparation were in line with the related provisions of “0101 tablet”stated in 2015 edition of Chinese Pharmacopoeia (part Ⅳ). TLC indicated that the chromatogram of the test sample showed the main spots of same color as the corresponding positions of the chromatogram of schizandrol A control ,Schisandra chinensis reference substance and raw material ,while the negative control has no interference. Content determination results shows that the average content of schizandrol A and total lignans in SCTL gastric floating tablets is 3.187,19.617 mg. It was preliminarily formulated that the content limitation of schizandrol A in one tablet should not be less than 2.50 mg,and the content of total lignans (calculated by schizandrol A )should not be less than 15.50 mg. CONCLUSIONS:The preparation technology of SCTL gastric floating tablets is stable ,feasible and controllable in quality.

      摘要:OBJECTIVE:To study the effects of costunolide on the proliferation ,migration and apoptosis of breast cancer SK-BR-3 cells and its mechanism. METHODS :SK-BR-3 cells in logarithmic growth period were collected and cultured with different concentrations (10,20,30,40,50 μmol/L)of costunolide for 24,48,72 h. Inhibitory rate of costunolide on cell proliferation was detected with CCK- 8. The cells were divided into blank control group and costunolide group (10,20,30 μmol/L). Hoechst 33258 fluorescence was used to observe the morphology and apoptosis of cells ,and apoptotic rate of cells were calculated. Cell scratch test was used to detect the migration ability of cells and calculate the migration rate. Western blotting was used to detect the relative expression level of Bcl- 2,Bax,Caspase-3 and Cleaved Caspase- 3 in cells. RESULTS :The proliferation of SK-BR-3 cells were significantly inhibited by costunolide (P<0.05 or P<0.01),and it shows a trend of concentration and time dependence. In the blank control group ,cells possessed clear contour ,regular shape and good adherence . Compared with blank control group,the number of cells were decreased significantly in 10,20,30 μmol/L costunolide groups,the cell structure was loose,the volume was reduced ,and the gap became larger ,and most of the cell contour disappeared and became round ,the cell adherence was poor ;cell migration rate and Bcl- 2 protein relative expression level were decreased significantly ,while apoptosis rate and the relative expression level of Bax ,Caspase-3 and Cleaved Caspase- 3 protein were significantly increased (P<0.05 or P<0.01). CONCLUSIONS : Costunolide can inhibit the proliferation and migration ,and induce apoptosis of human breast cancer SK-BR- 3 cells,mechanism of which may be through up-regulating the expression of Bax ,Caspase-3 and Cleaved Caspase- 3 while down-regulating the expression ofBcl-2.

      摘要:OBJECTIVE:To screen the components with better anti-inflammatory effect from Codonopsis Radix polysaccharide and explore the anti-inflammatory mechanism. METHODS :Using Codonopsis Radix as sample ,the crude polysaccharide (CP) was obtained by water extraction and alcohol precipitation. After CP was separated and purified by Fast Flow DEAE Sepharose , SephadexG-150 gel column and so on ,the components CP 1-2-1 and CP 3-1-1 were obtained and then characterized. The survival rate of RAW 264.7 cell was determined by MTT method after cultured with CP ,CP1-2-1 and CP 3-1-1(0.01,0.1,1,10,100 μg/mL)for 24 and 48 h respectively. The cells were divided into blank group (blank culture medium ),model group (1 μg/mL LPS)and low-,medium- and high-concentration groups of 3 kinds of Codonopsis Radix polysaccharide (1 μg/mL LPS+25,50, 100 μg/mL CP,CP1-2-1,CP3-1-1 solution). After cultured for 24 h,NO content in cell culture solution ,mRNA expressions of TLR4,IL-6,NF-κB and TNF-α in cells were determined. RESULTS:The sugar content of CP reached (92.20±0.73)%. CP 1-2-1 was a single neutral polysaccharide composed of fructose with a relative molecular weight of 25.8 kDa. CP 3-1-1 was an acidic heteropolysaccharide composed of arabinose ,rhamnose,galactose and galacturonic acid and so on ,with a relative molecular weight of 49.5 kDa. Cell survival rates were higher than 99%,after cultured with 3 kinds of polysaccharide for 24 and 48 h. Compared with blank group ,NO content in cell culture solution and mRNA expressions of TLR 4,IL-6,NF-κB and TNF-α in cells were increased significantly in model group (P<0.05 or P< . Compared with model group , NO content in cell 2016CFB412) culture solution of each administration group was significantly reduced,mRNA expressions of TLR 4,NF-κB,TNF-α,IL-6 in cells in 50,100 μg/mL CP group were significantly decreased ong1991@163.com (P<0.05 or P<0.01);mRNA expressi ons of TLR 4,NF-κB in cells in 25 µg/mL CP 1-2-1 group and mRNA expressions of TLR 4,NF-κB,TNF-α,IL-6 in cells in 50,100 µg/mL CP 1-2-1 group were significantly decreased (P<0.05 or P<0.01);mRNA expressionsof IL- 6 in cells in 25 µg/mL CP 3-1-1 group,mRNA expressions of NF-κB,TNF-α,IL-6 in cells in 50 µg/mL CP 3-1-1 group and mRNA expressions of TLR 4,NF-κB,TNF-α,IL-6 in cells in 100 µg/mL CP 3-1-1 group were significantly decreased (P<0.05 or P<0.01). CONCLUSIONS :CP,CP 1-2-1 and CP 3-1-1 all have certain anti-inflammatory activities. The mechanism may be related to inhibiting the generation and releasing of inflammatory cytokines such as TNF-α,IL-6 by inhibiting the activation of TLR 4/NF-κB pathway.

      摘要:OBJECTIVE:To study the effects of matrine on proliferation and collagen synthesis of rat hepatic stellate cells CFSC-8B activated by acetaldehyde ,and to investigate its possible mechanism. METHODS :CFSC-8B cells cultured in vitro were divided into blank control group ,model group ,positive control group (2.5 μmol/L colchicine)and matrine low ,medium and high concentration groups (30,60,120 μmol/L). Except for blank control group ,other groups were activated with 200 μmol/L acetaldehyde for 24 h;medicine groups were intervened with relevant medicine for 24 h(blank control group and model group were intervened with equal volume blank medium ). Survival rate of cell was detected by CCK- 8 assay. C ells were divided into blank control group ,model group ,positive control group (2.5 μmol/L colchicine),matrine medium and high concentration groups (60,120 μmol/L),then activated and treated with same method. Hydroxyprolin (Hyp)content in cell culture solution was tested by enzyme digestion. The contents of Col- Ⅰ and Col- Ⅲ in cell culture solution were determined by ELISA. mRNA expressionss of α-SMA,TGF-β1,TβR-І,TβR-Ⅱ,Smad3,Smad4 and Smad 7 in cells were detected by RT-PCR. The protein expressions of α-SMA,TGF-β1,TβR-Ⅰ,TβR- Ⅱ,Smad3,Smad4 and Samd 7 in cells were detected by Western blotting. RESULTS :Compared with blank control group ,survival rate of cells in model group was increased significantly (P<0.05);the contents of Hyp ,Col-Ⅰ and Col- Ⅲ in cell culture solution ,mRNA and its protein expressions of α-SMA,TGF-β1,TβR-Ⅰ,TβR-Ⅱ,Smad3,Smad4 in cells were increased significantly in model group (P<0.05),while the mRNA and protein expression of Smad 7 was decreased significantly(P<0.05). Compared with model group ,survival rate of cells ,the contents of Hyp ,Col-Ⅰ and Col- Ⅲ in cell culture solution,the mRNA and protein expressions of α-SMA and Smad 4 were decreased significantly in positive control group and matrine medium and high concentration groups (P<0.05), while the mRNA and protein expression of Smad 7 was WF-0099) increased significantly (P<0.05);the mRNA and proteinexpressions of TGF-β1,TβR-Ⅰ,TβR-Ⅱ and Smad 3 were decreased significantly in positive control group and matrine high concentration group (P<0.05). Compared with matrine medium concentration group ,all above indexes were improved significantly in matrine high concentration group (P<0.05). CONCLUSIONS :Matrine can suppress the proliferation and collagen synthesis of CFSC- 8B cells activated by acetaldehyde ,with a centain concentrlation dependence ,the mechanism of which may be associated with regulating the conduction of TGFβ/Smad signal pathway.

      摘要:OBJECTIVE:To know about the research status of health utility value obtained by mapping method in pharmacoeconomic evaluation ,and to provide reference for bibliometric study in pharmacoeconomic evaluation . METHODS : Using“Mapping method ”“Health utility value ”“Cost-utility”“Utility point system ”as Chinese and English keywords ,retrieved from CNKI ,Wanfang database ,PubMed,Medline,Ebsco,Ovid and Wiley database ,empirical journal documents published from the inception to Dec. 31st,2018 about using mapping method to obtain health utility value were collected. The bibliometrics was used to statistically analyze basic information of included literature ,the construction and test of the model ,the type of the best model and so on. RESULTS :The 124 included documents were all published in English journal. In the construction and testing of the mapping model ,the most frequently used econometric methods ,performance evaluation indicators and model testing methods were ordinary least squares (OLS),mean absolute error (MAE)and residual normality test ,application frequency of which were 97 times(31.60%),89 times(24.93%)and 62 times(21.09%). There are 117 articles that define the best mapping model ,of which 101 articles(86.32%)have the best direct mapping effect. Most of the non-utility measurement scales adopted specific scales (92 articles,77.97%),and a few literatures adopted the universal scale (26 articles,22.03%). The most utility measurement scales were 3-level European 5-dimensional health scale (79 articles,66.95%). CONCLUSIONS :The domestic empirical researches that use the mapping method to obtain health utility values need to be developed yet. A series of mapping models successfully developed by foreign scholars not only provide the feasibility of using non-utility measurement scales for cost-utility analysis ,but also provide more ideas for China to choose the corresponding econometric methods ,evaluation indicators and mapping methods in the empirical research of the mapping method in the future.

      摘要:OBJECTIVE:To systematically e valuate the effectiveness of inhaled corticosteroids combined with tiotropium bromide versus inhaled corticosteroids alone in the treatment of asthmatic-COPD overlap syndrome ,and to provide evidence-based reference for clinical use. METHODS :Retrieved from PubMed ,Web of Science ,Embase,Cochrane Library ,CBM,CNKI, VIP,Wanfang database ,during the establishment of the database to Nov. 2019,randomized controlled trials (RCT)about inhaled corticosteroids combined with tiotropium bromide (trial group )versus inhaled corticosteroids alone (control group )in the treatment of asthmatic-COPD overlap syndrome were collected. After data extraction of included literatures met inclusion criteria ,quality evaluation with modified Jadad scale ,Meta-analysis was performed for response rate ,forced expiratory volume in the first second (FEV1),forced vital capacity (FVC),FEV1/FVC,inspiratory fraction (IC/TLC),residual to total ratio (RV/TLC),asthma symptom(ACT)score,chronic obstructive pulmonary symptom (CAT)score,the times of acute exacerbations by Rev Man 5.3.0 software. RESULTS :A total of 25 RCTs were included ,involving 2 828 patients. The results of Meta-analysis showed that the response rate [RR =1.16,95%CI(1.10,1.22),P<0.001],FEV1[MD=0.44,95%CI(0.35,0.54),P<0.001],FVC [MD =0.70, 95%CI(0.46,0.95),P<0.001],FEV1/FVC [MD= 8.79,95%CI(6.22,11.37),P<0.001],IC/TLC [MD =4.93,95%CI(3.01, 6.85),P<0.001],RV/TLC [MD =-9.22,95%CI(-9.79,-8.66),P<0.001],ACT score [MD =5.38,95%CI(4.30,6.47), P<0.001],CAT score [MD =-3.67,95%CI(-4.89,-2.45),P<0.001] and the times of acute exacerbations [MD =-1.49, 95%CI(-2.82,-0.17),P=0.03] in trial group were significantly higher than control group ,with statistical significance. CONCLUSIONS:Compared with inhaled corticosteroids alone ,inhaled hormone combined with tiotropium bromide can improve the response rate and pulmonary function ,but increase the times of acute exacerbation of patients with asthmatic-COPD overlap syndrome.

      摘要:OBJECTIVE:To systematically e valuate the efficacy and safety of duloxetine in the improvement of pain symptoms of knee osteoarthritis (KOA),and to provide evidence-based reference for clinical treatment of KOA. METHODS :Retrieved from PubMed,Embase,Medline,Cochrane Library ,CNKI,VIP and Wanfang database ,during the establishment of the database to Sept. 2019,RCTs about duloxetine (trial group )vs. placebo (control group )in the improvement of pain symptoms of KOA were collected. After data extraction and quality evaluation using Cochrane system evaluator ’s manual 5.1.0,Meta-analysis was performed by using Stata 14.0 software for WOMAC total score ,WOMAC pain score ,WOMAC stiffness score ,WOMAC function score and BPI-S score ,as well as the incidence of adverse reaction such as dry mouth ,somnolence and nausea. RESULTS:A total of 6 RCTs were included ,involving 2 059 patients. Meta-analysis results showed that WOMAC total score [MD=-0.34,95%CI(-0.48,-0.20),P<0.05],WOMAC pain score [MD =-0.41,95%CI(-0.54,-0.29),P<0.05], WOMAC stiffness score [MD =-0.24,95% CI(-0.37,-0.12),P<0.05],WOMAC function score [MD =-0.43,95%CI (-0.55,-0.31),P<0.05],BPI-S score [MD =-0.38,95%CI(-0.48,-0.28),P<0.05] of trial group were significantly lower than those of control group ;the incidence of dry mouth [RR =3.55,95%CI(2.00,6.29),P<0.05],somnolence [RR =3.23, 95%CI(1.88,5.54),P<0.05] and nausea [RR =6.95,95%CI(2.99,16.15),P<0.05] in trial group were significantly higher than control group ,with significant difference. CONCLUSIONS :Duloxetine can relieve the pain and improve knee function in patients with KOA ,but it is necessary to pay attention to its adverse reactions.

      摘要:OBJECTIVE:To promote rational use of OTC in the family ,and to provide reference for the formulation of related policy by health administration department. METHODS :By stratified cluster sampling method ,using residents of 4 cities from Gansu province as objects ,face-to-face questionnaire household survey was conducted in Jan. 2019. The general information of the residents and their family self-medication behaviors were collected ,and the influential factors [tendency factors (including cognition of OTC and attitude towards self-medication ),contributing factors and strengthening factors ,and odds ratio (OR)was used to study the correlation between factors and results] of self-medication behaviors were analyzed by PRECEDE-PROCEED model. Epi Data 3.10 software was used to establish database ,and SPSS 16.0 software was adopted to analyze the family self-medication behavior of residents statistically. RESULTS :A total of 3 600 questionnaires were sent out ,and 3 111 valid questionnaires were recovered ,with an effective recovery rate of 86.4%. 81.8%(2 541/3 106)of the respondents had self- medication behavior in the past 1 year. The results of binary Logistic regression and χ2 test showed that female residents ,irregular daily life , no chronic disease ,low monthly family income ,new rural cooperative medical care and urban residents ’medical insurance were more inclined to self-medication. In the contributing factors ,the incidence of self-medication of residents with regular drugs in their home was 2.997 times higher than that those without reserves ;the convenience of self-medication (OR=1.376),the proximity of drugstores to home (OR=1.680),the credibility of drug quality (OR=1.355) had a facilitating effect on the occurrence of self-medication. In the strengthening factors ,the suggestions of relatives and friends (OR=3.388),books, newspapers and TV network advertisements (OR=2.043)had a strengthening effect on the occurrence of self-medication. The above factors had a significant effect on the residents ’self-medication(P<0.05). CONCLUSIONS :The family self-medication behavior of residents in Gansu province is common. The residents with higher cognitive level tend to self-medication ;the lower the drug price ,the more likely the self-medication behavior will occur. The factors contributing to the occurrence of self-medication are the regular drugs at home ,the proximity of drugstores to home ,and the credibility of drug quality. The suggestions of relatives and friends,online advertising and other factors are the strengthening factors of the occurrence of self-medication behavior.

      摘要:OBJECTIVE:To explore the role of clinical pharm acists in anti-infective therapy in patients with septic shock in the ICU. METHODS :A total of 180 patients with septic shock were retrospectively collected from ICU of Chongqing Qianjiang Central Hospital during Jan.-Dec. 2018. According to whether clinical pharmacists participated in anti-infective therapy in the whole process,the patients were divided into intervention group (92 cases)and control group (88 cases). Both groups were given routine treatment according to clinical diagnosis ;on this basis ,clinical pharmacists participated in the whole anti-infective therapy of the intervention group ,including participating in pharmaceutical rounds ,formulating anti-infective programs ,guiding nurses to implement anti-infective plan ,and providing pharmaceutical care for patients. The effectiveness ,safety and antibiotics clinical use indexes(such as proportion ,treatment course and cost of key monitored drugs )were evaluated in 2 groups. RESULTS :The cure rate of intervention group (98.91%)was significantly higher than that of control group (93.18%),while the incidence of ADR in the intervention group (8.70%)was significantly lower than control group (23.86%). The use of antibiotics types was more rational(the proportion of carbapenems used in intervention group and control group were 20.65%,36.36%;those of quinolones were 15.22%,28.41%,respectively);the duration of antibiotics use in the intervention group [ (15.18±2.25)d] was significantly shorter than control group [ (19.84±3.81)d]. The cost of antibiotics per capita [ (2 846.99±712.48)yuan] was significantly lower than control group [ (3 991.26±577.82)yuan],with statistical significance of all above (P<0.05 or P<0.01). CONCLUSIONS : Clinical pharmacists participating in the anti-infective treatment of ICU patients with septic shock and providing pharmaceutical care,can improve the anti-infective effect ,reduce adverse drug reactions and drug-induced diseases ,optimize the use of antibiotics and reduce medical cost.
