      摘要:OBJECTIVETo provide theoretical basis for the rational use of drugs in medical institutions, assist in improving the quality of pharmaceutical services, and thus meet clinical drug demands.METHODSAdopting consensus meetings, Liaoning Pharmaceutical Association,Jilin Pharmaceutical Association and Heilongjiang Pharmaceutical Association collaborated with clinical and pharmaceutical experts in the region to compile the expert consensus on off-label drug use in the three Northeastern provinces of China after many votes and discussions by collecting and collating the information related to off-label drug use in medical institutions from the three northeastern provinces of China,and referring to and citing off-label drug use stated in some expert consensus and medication catalog.RESULTSFinally, a total of 198 pieces of off-label drug use information for 70 drugs were included in the two sections of solid tumors and hematological diseases in Consensus of Experts on Drug Use beyond the Instructions in the Three Provinces of Northeast China.CONCLUSIONSConsensus of Experts on Off-label Drug Use in the Three Northeastern Provinces of Northeast China (solid tumors and hematology)offers a theoretical foundation for rational drug use in the treatment of solid tumors and hematological diseases within medical institutions,and has a positive significance in improving the effectiveness and safety of drug treatment.

      摘要:OBJECTIVETo evaluate the safety and suitability of traditional Chinese medicine prescriptions generated by generative artificial intelligence (AIGC), and to provide research ideas for empowering the traditional Chinese medicine industry with AIGC.METHODSUsing the 2020 edition of Chinese Pharmacopoeia and the 5th edition of Traditional Chinese Medicine as corpus, GPT-4 and the real-time networking model developed based on GPT-4 (referred to as the “networking model”) were used for deep learning. The clinical cases included in the consensus of traditional Chinese medicine experts in recent years were extracted manually to regenerate prescriptions based on diagnosis using the GPT-4 model and networking model; traditional Chinese medicine experts conducted blind evaluation and scoring of GPT-4 generated prescriptions, networking model generated prescriptions, and expert consensus prescriptions. At the same time, Turing testing was used to evaluate whether the GPT-4 model and networking model had the same ability as human intelligence.RESULTSThe average score of traditional Chinese medicine prescriptions generated by the GPT-4 model showed no statistically significant difference compared to manual prescriptions (P>0.05), while the average score of prescriptions generated by the networking model showed no statistically significant difference compared to traditional Chinese medicine prescriptions generated by the GPT-4 model (P>0.05). The proportion of model-generated prescriptions mistakenly judged as manual prescriptions in the Turing test was 51.11%.CONCLUSIONSThe traditional Chinese medicine prescriptions generated by the GPT-4 model have reached a certain level of safety and suitability, and the GPT-4 model has passed the Turing test. The introduction of AIGC in the diagnosis and treatment process may provide technical support for the rational use of clinical traditional Chinese medicine.

      摘要:OBJECTIVETo optimize the preparation technology of baicalin (BCN)-glycyrrhizic acid (GA) solid nanocrystals (BCN-GA-SN), to characterize them and investigate their in vitro release characteristics.METHODSAccording to the compatibility ratio of classic couplet medicinals “Scutellaria baicalensis-Glycyrrhiza uralensis”, the compatibility ratio of BCN and GA was determined as 6∶1 (m/m); BCN-GA nanosuspension was prepared by precipitation method combined with high-pressure homogenization method. The preparation technology of BCN-GA nanosuspension was optimized by using mean particle size and polydispersity index (PDI) as indexes and with types and dosage of stabilizers, stirring speed and time, high-pressure homogenization pressure and frequency as factors. The freeze-dried consolidation process of BCN-GA nanosuspension was optimized to prepare BCN-GA-SN using average particle size, PDI and redispersibility index (RDI) as indicators, with the type and dosage of freeze-dried protective agents as factors; then, the physicochemical properties and in vitro release of BCN-GA-SN were investigated.RESULTSThe optimal preparation technology of BCN-GA-SN was as follows: BCN-GA nanosuspension was prepared by using 15% sodium dodecyl sulfate as a stabilizer, stirring at 1 000 r/min for 15 minutes, and homogenizing at 100 MPa for 20 times; then, BCN-GA nanosuspension was freeze-dried and solidified with 5% mannitol (corresponding to the dosage of BCN). The average particle size of prepared BCN-GA-SN was (442.2±5.7) nm with PDI of 0.225±0.015 and RDI of 1.055±0.013. The prepared BCN-GA-SN presented as the irregularly spherical shape with more uniform size; the drug-loading amount of BCN in the nanocrystal was (62.5±0.7)%, and that of GA was (9.4±0.2)%; the in vitro release results showed that the cumulative dissolution of BCN-GA-SN was higher than that of the physical mixture of BCN and GA.CONCLUSIONSBCN-GA-SN is prepared successfully in this study with uniform particle size and even distribution, which can effectively improve the dissolution of BCN.

      摘要:OBJECTIVETo explore the mechanism of Yishen tongluo formula (YSTLF) in improving abnormal lipid metabolism based on the sterol regulatory element binding proteins (SREBPs) pathway.METHODSUsing C57BLKS/J (db/db) mice as model and C57BLKS/J (db/m) mice as normal control, the mechanism of 1, 2.5 and 5 g/kg YSTLF improving abnormal lipid metabolism of db/db mice was investigated by determining the liver coefficient, the contents of serum total cholesterol (TC), triglyceride (TG), low-density lipoprotein (LDL) and high-density lipoprotein (HDL), observing steatosis and lipid accumulation in liver tissue of mice, detecting the protein expressions of SREBP-1 and SREBP-2 as well as mRNA transcription levels of Srebp-1c, Srebp-2 and their downstream lipid metabolism-related target genes (Fasn, Acc1, Scd5, Fads1, Hmgcr, Dhcr24, Insig-1, Fdps) in liver tissue of mice. Using low-fat cultured human liver cancer cell HepG2 as an in vitro cell model for abnormal lipid metabolism, and 25-HC (SREBPs inhibitor, 10 μmol/L) as the control, the effects of 125, 250 and 500 μg/mL YSTLF on protein expressions of SREBP-1 and SREBP-2 as well as mRNA transcription of SREBP-1c, SREBP-2 and their downstream lipid metabolism-related target genes were investigated to verify the mechanism in vitro.RESULTS1, 2.5, 5 g/kg YSTLF significantly reduced the levels of TC, TG and LDL, the percentage of lipid droplet-positive region in liver tissue and liver coefficient, significantly down-regulated protein expressions of Pre-SREBP-1, n-SREBP-1, Pre-SREBP-2 and n-SREBP-2, and mRNA transcription of Srebp-1c, Srebp-2 and their downstream target genes in liver tissue, while significantly increased HDL level, with statistical significance (P<0.05 or P<0.01). In the cell experiment in vitro, the expressions of the above-mentioned proteins and genes in the cells treated with YSTLF at 125, 250 and 500 μg/mL for 24 hours were consistent with those in the animal experiment; there was no significant difference in the expressions of the above-mentioned proteins and genes between inhibitor control group and 250, 500 μg/mL YSTLF groups (P>0.05).CONCLUSIONSYSTLF can regulate the expression of transcription factor SREBPs, so as to inhibit the high expression of fatty acid and cholesterol synthesis-related genes, promote the degradation of TC and TG, improve the abnormality of lipid metabolism and inhibit lipid accumulation, thus playing the role of lipid-lowering.

      摘要:OBJECTIVETo optimize the extraction technology for the raw drugs of Sanse powder gel paste.METHODSSD rats were divided into blank group, model group, traditional technology group, water extraction group and ethanol extraction group, with 5 rats in each group. Anterior cruciate ligament transection was used to construct knee osteoarthritis model, and the pharmacodynamic effects of different extraction methods on arthritic rats were investigated. Analgesic experiments were conducted using cold and hot pain thresholds and pain mediators calcitonin gene-related peptide (CGRP), cyclooxygenase-2 (COX-2), substance P (SP), and prostaglandin E2 (PGE2) contents as indicators. HE staining was performed on the synovial membrane of rats to observe the degree of synovial cell proliferation, inflammatory infiltration and vascular invasion, and anti-inflammatory experiments were conducted using protein and mRNA expressions of interleukin-1β (IL-1β), tumor necrosis factor-α (TNF-α) and IL-6 as indicators. The analgesic and anti-inflammatory effects were compared among those groups. In the orthogonal test, ethanol dosage, extraction time and extraction times were used as evaluation factors, and the contents of casticin, strychnine and toxiferine were taken as evaluation indicators; comprehensive score was calculated. The validation experiments were carried out after optimizing the extraction technology of the raw drugs of Sanse powder gel paste.RESULTSCompared with the model group, the cold and heat pain thresholds of drug administration groups (except for the traditional technology group) were all increased significantly (P<0.05), while the contents of pain mediators CGRP, COX-2, SP and PGE2 were all decreased significantly (P<0.05). HE staining showed that inflammatory cell infiltration, fibrosis and collagen deposition were decreased in the administration groups; a small amount of capillary proliferation could be found; the protein and mRNA expressions of inflammatory factors such as IL-1β, IL-6 and TNF-α were decreased significantly in synovial tissue of rats in administration groups (P<0.05). Compared with the traditional technology group, most indicators of the ethanol extraction group were significantly reduced (P<0.05), and only heat pain threshold and mRNA expression of IL-6 in rats were decreased significantly in the water extraction group (P<0.05). The optimal extraction technology of the raw drugs of Sanse powder gel paste included suitable dose of Sanse powder, 8-fold 55% ethanol, heating reflux extraction for 90 minutes, extracting twice. The results of 3 times of verification experiments showed that the average contents of casticin, strychnine and toxiferine were 0.007%, 0.092%, and 0.214%, respectively; RSD were all less than 5%.CONCLUSIONSThe optimized extraction technology for the raw drugs of Sanse powder gel paste is stable and feasible, which can improve the efficacy of the preparation.

      摘要:OBJECTIVETo study the effects of isoliquiritigenin (ISL) regulating gut microbiota and repairing gut barrier function in model mice with non-alcoholic fatty liver disease (NAFLD), and to clarify its mechanism for improving NAFLD.METHODSThirty male C57BL/6J mice were randomly divided into the normal (ultrapure water), model group (ultrapure water), ISL group (100 mg/kg), with 10 mice in each group. Model group and ISL group were fed with high-fat diet for 19 weeks to establish NAFLD model; at the same time, the mice were given relevant medicine/ultrapure water intragastrically. The changes of body weight in mice were recorded, and liver index, white fat index and brown fat index were calculated. The pathological changes of liver tissue and colon tissue as well as lipid accumulation were observed in mice. The levels of total cholesterol (TC), triglyceride (TG), aspartate aminotransferase (AST) and alanine aminotransferase (ALT) in serum or liver were measured; the serum levels of interleukin-6 (IL-6), IL-1β and tumor necrosis factor-α (TNF-α) and the levels of IL-6, IL-1β and TNF-α mRNA expression in liver tissue were detected. Fecal samples underwent 16S rDNA sequencing analysis, and the effects of ISL on gut microbiota structure of mice were investigated. The expressions of gut mucosal barrier-related proteins (Claudin-4, Occludin and ZO-1) were determined in the colon tissue of mice.RESULTSCompared with model group, the body weight, liver index, the levels of TC in liver tissue and serum, the levels of AST and ALT in serum, the levels of IL-6, IL-1β and TNF-α in serum, and the mRNA expression of TNF-α in liver tissue were all decreased significantly in ISL group, while brown fat index was increased significantly. The inflammation and damage of liver tissue were significantly improved, and the NAFLD activity score and the proportion of lipid staining area were significantly reduced (P<0.05). ISL could significantly up-regulate the relative abundance of beneficial microbiota (norank_f_Muribaculaceae, Odoribacter, Ruminiclostridium, etc.) and the expressions of intestinal barrier function-related proteins, but could significantly down-regulate the relative abundance of harmful bacteria (Desulfovibrio, norank_f_Lachnospiraceae, unclassified_p_Firmicutes), and could repair intestinal barrier.CONCLUSIONSISL could significantly delay the progress of NAFLD, the mechanism of which may be associated with regulating gut microbiota and improving gut barrier function.

      摘要:OBJECTIVETo explore the role and underlying mechanism of tournefolic acid B (TAB) on the improvement of glucose metabolism and renal function in diabetic nephropathy (DN) model mice.METHODSDN model mice were established by high-fat diet combined with streptozotocin, and then randomly divided into model group, positive control group (vitamin E, 20 mg/kg), TAB low-dose, medium-dose and high-dose groups (1, 2, 4 mg/kg), with 12 mice in each group; normal control group was given regular diet. Each group was given relevant medicine or normal saline intragastrically, once a day, for 4 consecutive weeks. The glucose metabolic function was estimated by fasting blood glucose, glucose tolerance test, insulin tolerance test and serum insulin concentration. The renal coefficients and biochemical indicators related to renal function [serum uric acid, blood urea nitrogen, creatinine levels, and ratio of urine microalbumin to creatinine] were detected in mice; the contents of biochemical indicators related to oxidative stress [superoxide dismutase (SOD), glutathione peroxidase (GSH-Px), malondialdehyde (MDA), 8-hydroxydeoxyguanosine (8-OHdG)] were determined in renal tissue of mice; the pathological morphology of renal tissue was observed; the expressions of extracellular matrix (ECM) deposition related factors [transforming growth factor β1 (TGF- β1), fibronectin (Fn), type Ⅳ collagen (Col Ⅳ)] and protein kinase B (Akt)/nuclear factor erythroid 2-related factor 2 (Nrf2)/heme oxygenase-1 (HO-1) pathway related proteins were determined in renal tissue of mice.RESULTSCompared with normal control group, fasting blood glucose, area under glucose tolerance curve, area under insulin tolerance curve, serum insulin content, the levels of uric acid, urea nitrogen and creatinine and ratio of urinary microalbumin to creatinine in serum, the contents of MDA and 8-OHdG and the protein expressions of TGF-β1, Fn and Col Ⅳ were increased significantly in model group (P<0.05), while the contents of SOD, GSH-Px and the protein expressions of p-Akt, Nrf2, HO-1 in renal tissue were decreased significantly (P<0.05); the significant thickening of the basement membrane, accumulation of mesangial matrix, glomerulosclerosis and interstitial fibrosis of the renal tubules were all found. Compared with model group, above indexes of mice were all reversed significantly in TAB groups (P<0.05), and pathological changes were alleviated in a dose-dependent manner.CONCLUSIONSTAB can improve blood glucose metabolism and kidney function and alleviate renal tubulointerstitial fibrosis in DN model mice, the mechanism of which may be associated with activating the Akt/Nrf2/HO-1 signaling pathway and suppressing ECM deposition.

      摘要:OBJECTIVETo establish the HPLC fingerprint of Xintongshu spray, determine the contents of identified components, and investigate the transferring patterns of the index components of decoction pieces, intermediates and spray, so as to provide scientific reference for technology management and quality control of Xintongshu spray.METHODSHPLC fingerprints of 13 batches of Xintongshu spray were established by the Similarity Evaluation System for Chromatographic Fingerprints of TCM (2012 edition), and common peaks were identified; the contents of identified components were determined by HPLC. The paeonol in Moutan Cortex and ferulic acid in Chuanxiong Rhizoma were used as index components to investigate the transferring patterns of them in decoction pieces, intermediates and spray.RESULTSThere were a total of 33 common peaks in the fingerprints of 13 batches of Xintongshu spray, and the similarities were more than 0.994. Eight components were identified, i.e. gallic acid (peak 5), oxypaeoniflorin (peak 9), chlorogenic acid (peak 10), caffeic acid (peak 14), paeoniflorin (peak 17), ferulic acid (peak 21), senkyunolide Ⅰ (peak 27) and paeonol (peak 31). The contents of 8 components ranged from 0.590 3-0.719 7, 0.565 7-0.851 3, 0.279 4-0.368 1, 0.080 6-0.106 1, 1.922 5-3.033 5, 0.151 3-0.191 6, 0.250 6-0.336 0, 3.056 7-4.161 0 mg/mL, respectively. The average transfer rates of paeonol and ferulic acid from decoction pieces to sprays were 63.76% and 38.06%, respectively. It was also found that the process in which the loss of paeonol was more than 30% was the extraction by percolation and negative pressure concentration of Moutan Cortex. The process in which the loss of ferulic acid was more than 50% was the steam distillation extraction process of Chuanxiong Rhizoma.CONCLUSIONSThe established HPLC fingerprint and content determination method of Xintongshu spray are reproducible and specific. The key processes that cause a decrease in the average transfer rates of the index components are the extraction by percolation and negative pressure concentration of Moutan Cortex and steam distillation extraction of Chuanxiong Rhizoma.

      摘要:OBJECTIVETo investigate the effects and mechanism of atractylodin on inflammatory injury of periodontal tissue and alveolar bone loss in periodontitis rats.METHODSA total of 144 SD rats were divided into control group (intragastric and intraperitoneal injection of normal saline), model group (intragastric and intraperitoneal injection of normal saline), atractylodin low-dose, medium-dose and high-dose groups (intraperitoneal injection of 6.665, 13.33, and 26.66 mg/kg atractylodin), metronidazole group (positive control group, intragastric injection of 0.05 g/kg metronidazole, intraperitoneal injection of normal saline), AMD3100 [stromal cell-derived factor-1 (SDF-1)/CXC chemokine receptor-4 (CXCR4) pathway inhibitor] group (intragastric injection of 1 mg/kg AMD3100, intraperitoneal injection of normal saline), atractylodin high-dose+AMD 3100 group (intraperitoneal injection of 26.66 mg/kg atractylodin, intragastric injection of 1 mg/kg AMD3100), with 18 rats in each group. Except for the control group, all other groups of rats were inoculated with Porphyromonas gingivalis to construct a periodontitis model. After successful modeling, they were given relevant medicine or normal saline, once a day, for 4 consecutive weeks. The gingival index of rats was detected; the levels of interleukin-6 (IL-6) and tumor necrosis factor α (TNF-α) in rat serum were also determined; alveolar bone resorption, periodontal histopathologic changes and the number of osteoclasts were detected by methylene blue staining, HE staining and TRAP staining, respectively. The expressions of osteoprotegerin (OPG), receptor activator of NF-κB ligand (RANKL), SDF-1 and CXCR4 proteins were determined.RESULTSCompared with the control group, serious pathological injury of periodontal tissue was found in the model group, the gingival index, the levels of IL-6 and TNF-α, alveolar bone absorption value, the number of osteoclasts, and the expression of RANKL protein were all increased significantly (P<0.05), while the expressions of OPG, SDF-1 and CXCR4 proteins were decreased significantly (P<0.05). Compared with the model group, pathological injury of periodontal tissue in rats was reduced; the gingival index, the levels of IL-6 and TNF-α, alveolar bone resorption value, osteoclast number and RANKL protein expression were decreased significantly, while protein expressions of OPG, SDF-1 and CXCR4 were increased significantly in atractylodin low-dose, medium-dose and high-dose groups and metronidazole group (P<0.05). The change trend of corresponding indexes in the AMD3100 group was opposite to the above (P<0.05). AMD3100 attenuated the inhibitory effect of high-dose atractylodin on inflammatory response and alveolar bone loss in rats with periodontitis (P<0.05).CONCLUSIONSAtractylodin may improve the inflammatory response and alveolar bone loss in periodontitis rats by activating the SDF-1/CXCR4 signaling pathway.

The column temperature was 30 ℃, the injection volume was 5 μL. The detection wavelength of matrine was 211 nm, and the other components’ detection wavelength was 283 nm. The similarity evaluation of HPLC fingerprints for 10 batches of Jianpi huayu decoction was performed by using the Similarity Evaluation System of Chromatographic Fingerprint of Traditional Chinese Medicine (2012 edition). The contents of chlorogenic acid, vanillic acid, p-coumaric acid, ferulic acid, hesperidin, quercetin, bergapten and matrine in the samples were determined by HPLC.RESULTSHPLC fingerprint of Jianpi huayu decoction was established. A total of 27 common peaks were identified, and 8 components were identified. The similarity between 10 batches of samples and the control map ranged from 0.942-0.999. RSDs of precision, repeatability and stability tests were less than 3% (n=6). The average recoveries of chlorogenic acid, vanillic acid, p-coumaric acid, ferulic acid, hesperidin, quercetin, bergapten and matrine were 99.48%, 101.32%, 101.18%, 100.79%, 101.12%, 99.19%, 99.81% and 102.46%, respectively; RSDs were 1.34%, 0.93%, 1.90%, 1.84%, 0.54%, 1.53%, 1.33% and 1.01%, respectively (n=6). The contents were 0.021-0.061, 0.025-0.034, 0.116-0.295, 0.006-0.062, 0.014-0.053, 0.017-0.026, 0.014-0.027 and 14.05-24.11 mg/g, respectively.CONCLUSIONSThe established fingerprint and content determination method can provide a reference for the quality control and subsequent preparation development for Jianpi huayu decoction.

      摘要:OBJECTIVETo study the effect and potential mechanism of eriodictyol on non-alcoholic fatty liver disease (NAFLD).METHODSSixteen C57BL/6J mice were randomly divided into control group, NAFLD model group, and eriodictyol low-dose and high-dose groups (50, 100 mg/kg), with 4 mice in each group. Except for control group, the other groups were fed with high fat diet to induce NAFLD model. After four weeks of preprocessing, they were given relevant medicine intraperitoneally (0.01 mL/g), once a day, for 6 consecutive weeks. The body weight and liver weight of mice were measured, and the pathological damage of liver tissue in mice was observed. The levels of aspartate aminotransferase (AST), alanine aminotransferase (ALT), and triglycerides (TG) in serum, as well as the protein expressions of nuclear factor-erythroid 2-related factor 2 (Nrf2) and heme oxygenase-1 (HO-1) in liver tissue were determined. In vitro NAFLD model was established by using 0.5 mmol/L oleic acid (OA) in HepG2 cells. Normal control group, NAFLD model group and eriodictyol low-, medium- and high-concentration groups (50, 100, 150 μmol/L) were set up. HepG2 cells in drug groups were treated with eriodictyol for 24 h at the time of modeling. The lipid deposition was observed in cells, and the levels of TG, malondialdehyde (MDA) and reactive oxygen species (ROS) as well as the phosphorylation levels of the mitogen-activated protein kinase (MAPK) signal pathway related proteins [extracellular signal-regulated kinase (ERK), c-Jun N-terminal kinase (JNK)] and the protein expressions of Nrf2 and HO-1 were all determined.RESULTSIn the in vivo experiment, compared with the NAFLD model group, the body weight, liver weight, the serum levels of AST, ALT and TG were all decreased significantly in eriodictyol low- and high-dose groups (except for serum level of AST in eriodictyol low-dose group) (P<0.01); liver lipid deposition was reduced significantly and the protein expressions of Nrf2 and HO-1 in liver tissues were further up-regulated (P<0.01). In the in vitro experiment, compared with the NAFLD model group, the lipid deposition in hepatocytes was reduced in eriodictyol low-, medium- and high-concentration groups (P<0.01), and the levels of ROS, MDA and TG were down-regulated (P<0.05 or P<0.01); the phosphorylation levels of ERK and JNK were significantly down-regulated (P<0.01), while the protein expressions of Nrf2 and HO-1 were up-regulated significantly (P<0.01).CONCLUSIONSEriodictyol can inhibit MAPK signaling pathway and activate Nrf2/HO-1 signaling pathway to alleviate NAFLD.

      摘要:OBJECTIVETo study the improvement effect and mechanism of Gastrodia elata active ingredient 3,4-dihydroxybenzaldehyde (3,4-DD) on oxygen-glucose deprivation/reoxygenation(OGD/R) injury in rat primary brain microvascular endothelial cells (BMECs)-rat adrenal chromaffin cells PC12 co-culture system.METHODSThe co-culture model of BMECs and PC12 cells was replicated in the Transwell chamber, and divided into control group, model group, butylphthalide group (positive control group, 0.1 mmol/L) and 3,4-DD group (0.1 μmol/L). OGD/R injury model of the co-culture system was induced in those groups except for the control group. After preventively intervention in BMECs with relevant medicine or culture medium for 24 h, cell transendothelial electronic resistance (TEER) value, lactate dehydrogenase (LDH) activity, brain-derived neurotrophic factor (BDNF) level and mRNA expressions of TrkB, Plc-γ, Map-2, GAP-43 in PC12 cells was detected.RESULTSCompared with the control group, TEER of the co-culture model, LDH activity and BDNF level of PC12 cells were decreased significantly in the model group (P<0.01), while mRNA expressions of TrkB, Plc-γ, Map-2 and GAP-43 in PC12 cells were increased significantly (P<0.01). Compared with the model group, TEER of the co-culture model, LDH activity, BDNF level, and the mRNA expressions of TrkB, Plc-γ, Map-2 and GAP-43 in PC12 cells were increased significantly in the 3,4-DD group and butylphthalide group (P<0.05 or P<0.01).CONCLUSIONS3,4-DD can relieve the damage of neuronal OGD/R by acting on BMECs, the mechanism of which may be associated with activating the BDNF/TrkB signaling pathway.

      摘要:OBJECTIVETo evaluate the cost-effectiveness of pembrolizumab combined with chemotherapy versus placebo combined with chemotherapy in the first-line treatment of advanced or unresectable biliary tract carcinoma (BTC) from the perspective of China’s health system.METHODSA partitioned survival model was constructed based on the KEYNOTE-966 study data. The simulation period was 21 days, and the simulation time was the patient’s whole life. Using quality-adjusted life year (QALY) as the output indicator, the cost-utility analysis method was used to evaluate the cost-effectiveness of the two schemes mentioned above. Univariate and probabilistic sensitivity analyses were performed to verify the results of the basic analysis, and to explore the cost-effectiveness under the scenario of drug donation scheme.RESULTSThe basic analysis showed that both the cost and effectiveness of the pembrolizumab group were higher than those of the placebo group, and the incremental cost-effectiveness ratio (ICER) was 3 909 359.78 yuan/QALY, which was higher than the willingness-to-pay (WTP) threshold of 3 times 2022 gross domestic product (GDP) per capita (257 094 yuan), indicating no cost-effectiveness. The results of univariate sensitivity analysis showed that the utility discount rate, the utility value of progression-free survival (PFS) status, the cost discount rate, and the cost of pembrolizumab had a great influence on ICER. Probabilistic sensitivity analysis verified the robustness of the results of basic analysis, and concluded that when the WTP threshold was greater than 1 500 000 yuan/QALY, the pembrolizumab group became cost-effective. The results of the scenario analysis showed that considering the drug donation scheme of pembrolizumab for low-income people, although its treatment cost was significantly reduced, it was still not cost-effective.CONCLUSIONSAt the WTP threshold of 3 times China’s GDP per capita in 2022, pembrolizumab combined with chemotherapy is not cost-effective compared with placebo combined with chemotherapy for advanced or unresectable BTC.

      摘要:OBJECTIVETo provide references for the clinical safe use of axitinib.METHODSAdverse drug event (ADE) data for axitinib were collected from the US FDA Adverse Event Reporting System (FAERS) database from the first quarter of 2012 to the fourth quarter of 2022. The data were mined and analyzed by utilizing the ratio-of-reporting-ratio (ROR) method and comprehensive standard method of the United Kingdom’s Medicines and Healthcare Products Regulatory Agency (MHRA) of proportional imbalance measurement.RESULTSA total of 13 962 reports of axitinib-related ADEs were obtained, with patients’ age concentrated in 65-85 years (43.25%), gender predominantly male (65.23%), country of reporting predominantly US (60.01%), and serious ADE outcomes mostly hospitalization or prolonged hospitalization (31.51%). A total of 172 ADE risk signals were detected, involving 18 system and organ classifications (SOC), mainly systemic diseases and various reactions at the site of administration (3 749 cases, 30.84%) and gastrointestinal system diseases (2 067 cases, 17.00%). ADE risk signals that occurred more frequently were generally consistent with the drug instruction, such as diarrhea, fatigue, and hypertension; new ADE risk signals requiring clinical attention were death, immune-mediated nephritis, and PT signals contained in the SOC of various benign, malignant, and tumors of undetermined nature (including cysts and polyps).CONCLUSIONSFor ADEs that occur frequently with axitinib and are already contained in the drug instruction (e.g. hypertension, diarrhea), they should be adequately evaluated before administration, especially for patients with combined use of immune checkpoint inhibitors and patients with underlying hypertension; for ADEs with stronger signals and newer ADEs (e.g. death, disease progression, tumor progression), the patient’s disease progression should be closely monitored during the treatment period for potentially fatal ADEs; for its rare ADEs (e.g. immune-mediated nephritis, scrotal ulcer, non-infectious encephalitis), clinical validation should be further strengthened.

      摘要:OBJECTIVETo observe the efficacy and safety of ticagrelor combined with Compound xueshuantong capsules in the treatment of unstable angina pectoris.METHODSTotally 120 patients with unstable angina pectoris with deficiency of Qi and Yin combined admitted to Sanmenxia Hospital of Traditional Chinese Medicine from January 2021 to December 2022 were randomly divided into clopidogrel group (group A), ticagrelor group (group B) and combined medication group (group C), with 40 patients in each group. In addition to conventional treatment, group A was given clopidogrel orally; group B was given ticagrelor orally; group C was given ticagrelor and Compound xueshuantong capsule orally. After 12 weeks of treatment, the clinical efficacy, frequency of angina attacks, coagulation function indicators, cardiac function indicators, traditional Chinese medicine syndrome efficacy, and the incidence of adverse cardiovascular events and bleeding incidence were evaluated in the 3 groups.RESULTSThe total effective rates of group A, group B and group C were 77.5%, 85.0% and 90.0%, respectively. The incidence of adverse cardiovascular events and bleeding events were 7.5%, 7.5% and 5.0% in the respective groups, with no statistically significant difference (P>0.05). The frequency of angina attacks, duration of angina attacks and duration of dyspnea were significantly reduced or shortened in all 3 groups after treatment compared to before treatment (P<0.05). The frequency of angina attacks in group C was significantly lower than that in groups A and B after treatment (P<0.05). The levels of fibrinogen (FBG) and D-dimer in all 3 groups were significantly lower after treatment compared to before treatment (P<0.05); group A had significantly higher levels of FBG and D-dimer compared to group B and C (P<0.05). The left ventricular end-diastolic diameter and left ventricular end-systolic diameter in all three groups were significantly shorter after treatment, and the left ventricular ejection fraction was significantly higher compared to before treatment (P<0.05), but there were no statistically significant differences among those groups (P>0.05). The total effective rates of traditional Chinese medicine syndrome efficacy in groups A, B and C were 67.5%, 80.0% and 87.5%, respectively, with group C being significantly higher than group A (P<0.05).CONCLUSIONSIn addition to conventional treatment, ticagrelor combined with Compound xueguantong capsules can more significantly reduce the frequency of angina attacks in patients with unstable angina pectoris, reduce the levels of FBG and D-dimer, improve traditional Chinese medicine syndrome efficacy, and do not increase the risk of bleeding.

      摘要:OBJECTIVETo provide reference for clinically safe and rational drug use through mining and analyzing adverse drug event (AE) signals induced by valproic acid (VPA).METHODSReporting Odds Ratio (ROR) and Bayesian Confidence Propagation Neural Network (BCPNN) methods of Measures of Disproportionality were performed to mine and analyze the data of VPA-related AE reports in the US FDA Adverse Event Reporting System (FAERS) database from the first quarter of 2013 to the fourth quarter of 2022.RESULTSA total of 1 253 (ROR) and 1 109 (BCPNN) valid signals of preferred terms (PT) were obtained after data processing by the two analysis methods, involving 27 system organs (SOC), mainly focusing on nervous system disorders, psychiatric disorders, general disorders and administration site conditions. Signals that did not appear in the instruction were associated with 2 SOCs: ear and labyrinth disorders, infections and infestations.CONCLUSIONSAs a first-line broad-spectrum anti-epileptic drug, attention should also be paid to eye toxicity and infection risk in the clinical application in addition to paying attention to common adverse events in the instruction.

      摘要:OBJECTIVETo investigate the clinical efficacy and safety of herombopag combined with recombinant human thrombopoietin (rhTPO) in the treatment of primary immune thrombocytopenia (ITP) in the real world.METHODSA retrospective study was conducted on the patients diagnosed with ITP in the Second Affiliated Hospital of Bengbu Medical College from January 2021 to December 2022. Among them, 98 patients who were treated with a combination of herombopag and rhTPO were included in the observation group, and 157 patients who were treated with rhTPO alone were included in the control group. The changes in platelet count, clinical efficacy, bleeding, platelet transfusion rate and adverse drug reactions before and after treatment were observed and compared between the two groups.RESULTSSince the 8th day of treatment, there was a statistically significant difference in platelet count between the two groups [(61.04±13.46)×109 L-1 in observation group, (52.11±12.06)×109 L-1 in control group] (P<0.05), and there also was a statistically significant difference in the peak and stable values of platelet count between the two groups (P<0.05). The total effective rates of the observation group and the control group were 79.59% and 66.88%, with cumulative response rates of 81.32% and 68.68%, and median response durations of 8 days and 10 days, respectively; these differences were statistically significant (P<0.05). During the treatment period, the bleeding rates of the observation group and control group were 3.06% and 8.28% (P<0.05), bleeding events were categorized as grade 1 or 2, and platelet transfusion rates were 31.63% and 40.76%; the differences in bleeding rates and platelet transfusion rates between the two groups was statistically significant (P<0.05). The incidences of adverse drug reactions in the two groups were 11.22% and 9.55%, respectively, with no statistically significant difference (P>0.05), and no moderate to severe adverse drug reaction was found.CONCLUSIONSThe combination of herombopag and rhTPO can significantly increase platelet levels and response rate, and reduce bleeding rate and platelet transfusion rate in ITP patients, with good safety.

      摘要:OBJECTIVETo systematically reevaluate (umbrella review) the systematic review/meta-analysis of Tripterygium glycosides (TG) in the treatment of diabetic kidney disease (DKD), in order to provide a higher quality evidence-based reference for TG in the treatment of DKD.METHODSThe systematic reviews/meta-analysis of TG in the treatment of DKD were searched from CNKI, Wanfang, VIP, CBM, PubMed, Cochrane Library and Embase. The PRISMA 2020 statement, the AMSTAR 2 scale and the GRADE tool were used to evaluate the quality of the report, the quality of the methodology, and the quality of the evidence, respectively. The quantitative results of the included systematic review/meta-analysis were analyzed comprehensively.RESULTSA total of 18 systematic reviews/meta-analyses were included. PRISMA 2020 stated that 3 reports were complete, 13 reports had partial information defects, and 2 reports had serious information defects. The results of the AMSTAR 2 scale evaluation showed that 4 literature had low methodological quality, and 14 literature had very low methodological quality. GRADE tool evaluation results showed that there were 106 outcome indicators, including 34 intermediate-quality evidence accounted for 32.1%, 51 poor-quality evidence accounted for 48.1%, 21 very poor-quality evidence accounted for 19.8%, and there was no high-quality evidence. Comprehensive analysis of quantitative results of various outcome indicators showed that TG had definite improvement effects on the total effective rate of DKD, 24-hour urinary protein quantity and serum albumin, and the adverse drug reactions were different in every study.CONCLUSIONSThe efficacy of TG in the treatment of DKD is relatively accurate, safety still needs to be paid attention to, and future studies with larger sample size need to be verified.

      摘要:OBJECTIVETo provide reference for strengthening the quality of pharmaceutical care in retail chain pharmacies and the professional capabilities of licensed pharmacists, and to meet the public’s pharmaceutical care needs.METHODSA combination of questionnaire survey and informant interview was used to investigate the general information of licensed pharmacists in retail chain pharmacies in Guangdong province, the implementation of pharmaceutical care, the competence and knowledge status of the surveyed licensed pharmacists, and the factors affecting the development of pharmaceutical care. RESULTS &CONCLUSIONSA total of 444 questionnaires were sent out and 326 effective questionnaires were collected, with an effective recovery rate of 73.42%. Among 326 surveyed licensed pharmacists, a college degree or higher accounted for 40.18%, and the pharmacy major or related accounted for 79.75%. A total of 185 (56.75%) of the surveyed licensed pharmacists indicated that the retail chain pharmacies where they worked had established specified areas of pharmaceutical care; 320 (98.16%), 137 (42.02%) and 181 (55.52%) of the surveyed licensed pharmacists indicated that the retail chain pharmacies where they worked could provide services such as rational medication guidance for patients, decocting traditional Chinese medicines and establishing health records, etc. The self-assessment scores of the surveyed licensed pharmacists were significantly higher than 3 (median) for competence and knowledge level, but the self-assessment scores for medication therapy management competence and pharmaceutical care practice were relatively low. The surveyed licensed pharmacists believed that the main factors affecting the development of pharmaceutical care in retail chain pharmacies included patients’ lack of trust or cooperation (196, 60.12%), sales performance requirements (170, 52.15%), and heavy daily workload (165, 50.61%). It is recommended that relevant authorities expedite the legislative process of the Pharmacist Law, optimize the continuing education system for licensed pharmacists, and implement a star-rating evaluation system for pharmaceutical care capabilities in pharmacies, so as to encourage retail chain pharmacies and licensed pharmacists to pursue a more specialized development path. At the same time, pharmaceutical retail chain enterprises should shift their business mindset, increase investment in pharmaceutical care, enhance the professional atmosphere in the pharmacies, optimize the compensation structure for licensed pharmacists, and elevate the pharmaceutical care capabilities of both the pharmacies and licensed pharmacists to fulfill the pharmaceutical care needs of the public.

      摘要:erotonin-selective reuptake inhibitor (SSRI) is the first-line drug for treating depression. SSRI mainly include fluoxetine, paroxetine, sertraline, fluvoxamine, citalopram and escitalopram, etc. SSRI has dual impact on bone metabolism. Short-term use of SSRI may have a positive impact on bone, but long-term use may lead to bone problems. This article summarizes the effects and mechanisms of SSRI on bone metabolism, indicating that SSRI can affect bone formation, bone resorption, mesenchymal stem cell differentiation, and regulate the expression of inflammatory cytokines. The impact of SSRI on bone metabolism can be achieved by affecting classical signaling pathways such as cAMP/PKA/CREB, Wnt/β-catenin, BMP/Smad, OPG/RANKL/RANK, and through centrally mediated effects.

      摘要:Osteoporosis is an important cause of bone weakness and susceptibility to fractures. Anti-osteoporosis drugs of Western medicine cannot reverse its progression, and can only reduce the loss of bone density; long-term use of them is accompanied by certain adverse reactions. Traditional Chinese medicine focuses on syndrome differentiation and holistic approach, which can make up for the shortcomings of Western medicine’s treatment. The mammalian target of rapamycin (mTOR) signaling pathway is involved in the growth, proliferation, and differentiation of bone cells, and is closely related to the occurrence and development of osteoporosis. In recent years, various traditional Chinese medicine monomers (such as flavonoids, polysaccharides, alkaloids, etc.) and traditional Chinese medicine formulas (such as Bushen huoxue decoction, Liuwei dihuang pills, Erzhi pills, etc.) have been proven to promote bone formation, inhibit bone resorption, enhance bone cell autophagy, and delay the progression of osteoporosis by regulating the mTOR signaling pathway. Therefore, the article summarizes the traditional Chinese medicine monomer and formula that intervene in the mTOR signaling pathway for the treatment of osteoporosis, in order to provide medication ideas for the traditional Chinese medicine treatment of osteoporosis.

      摘要:Pulmonary fibrosis is the end-stage manifestation of a large class of lung diseases characterized by fibroblast proliferation and accumulation of a large amount of extracellular matrix accompanied by inflammatory injury and tissue structure destruction. Studies have shown that flavonoids have anti-pulmonary fibrosis effects through multiple paths, including dihydromyricetin, morin and fisetin can inhibit fibroblast differentiation; Trollius altaicus flavonoids, hesperidin and linarin can play an anti-inflammatory role; total flavonoids from Scutellaria baicalensis, scutellarin and chrysin can inhibit epithelial-mesenchymal transition; total flavonoids of Litchi chinensis, diosmin and silybin can play an anti-oxidative stress role; quercetin, baicalin and apigenin can regulate autophagy; total flavonoids of Oxytropis falcata, calycosin and dihydroquercetin can regulate apoptosis; naringin, scutellarin and total flavonoids of Dracocephalum moldavica can inhibit pyroptosis, thus exerting anti-pulmonary fibrosis effects.
