摘要:OBJECTIVETo establish a standardized management system for emergency medicines in medical institutions of Sichuan Province, so as to improve the timeliness, safety and accessibility of emergency medicine use.METHODSA multidisciplinary expert team consisting of specialists in emergency medicine, pharmacy and nursing was set up. In accordance with evidence-based principles, relevant domestic and international policies, guidelines and literature were systematically sorted out. Combined with the results of investigations on the current situation of medical institutions at all levels within the province, multiple rounds of thematic seminars of the steering expert group were organized. The Delphi method was applied to carry out two rounds of expert questionnaire consultation. After item-by-item revision and final expert review, the Expert Consensus on the Management of Emergency Medicines in Medical Institutions of Sichuan Province (hereinafter referred to as the Consensus) was finally developed.RESULTS&CONCLUSIONSThis Consensus constructs a three-tier catalog allocation model of “basic standard configuration + specialty adaptation + emergency reserve”. A set of standardized management specifications were formulated covering the selection, stock reserve, storage and maintenance, clinical application, supply and allocation, quality control and training, and informatization management of emergency medicines. The Consensus can provide a practical basis for the standardized, homogeneous and refined management of emergency medicines in medical institutions of Sichuan Province, and help enhance the capacity of medication guarantee and the level of medical safety management for the treatment of critical and severe patients across the province.

摘要:OBJECTIVETo provide evidence for further improving the rules governing the renewal round of national centralized drug procurement with volume-based purchasing (VBP) program for insulin.METHODSBased on publicly available policy documents, selection outcomes, and interview data, policy text analysis, descriptive statistical analysis, the Wilcoxon signed-rank test, and thematic analysis were used to compare the procurement rules and selection outcomes between the initial and renewal rounds of the national VBP programs for insulin, and to examine the mechanisms linking rule adjustments to changes in selection outcomes.RESULTSCompared with the initial round, the renewal round retained the six procurement groups and 11 bidding units, reduced the maximum valid bid price by 40%, and introduced competitive bidding selection, revival selection, and a price line for Category A products. A total of 42 products were selected in the initial round and 49 in the renewal round, including 37 that remained selected, 5 that exited, and 12 that were newly selected in the renewal round. Within the same procurement groups, the price range decreased significantly in the renewal round compared with the initial round (P=0.031), whereas the difference in average price between the two was not statistically significant (P=0.063). Domestic manufacturers were more likely to enter the A1 category, whereas multinational manufacturers were more likely to remain in categories B/C. Mechanism analysis suggested that the price line shaped the boundary of selected prices, while the classification and volume-allocation rules linked manufacturers’ bidding decisions to expected procurement volumes, potentially altering bidding incentives and corresponding to changes in the distribution of selection outcomes.CONCLUSIONSThe renewal round of the insulin VBP program established an interrelated mechanism involving price boundaries, volume-based incentives, and supply constraints through the use of price lines, product classification, and contracted volume allocation. These rule adjustments corresponded to changes in the structure of selected products, price distributions, and manufacturer classifications. Future policy refinement should strengthen the coordination among reasonable price ranges, volume-based incentives, and supply performance.

摘要:OBJECTIVETo provide references for medical institutions’ outpatient pharmacies to increase the scanning rate, scanning efficiency and scanning accuracy of drug traceability codes.METHODSA total of 4 157 unscanned traceability code records from the first two‑week period of June 2025 were selected from the outpatient pharmacy of our hospital. Pareto chart was adopted to analyze the reasons for unscanned codes, and the Objective-Rules-Training-Check-Culture (ORTCC) model was utilized to carry out fine management. The traceability code scanning rate, pharmacists’ work efficiency (number of prescriptions dispensed per pharmacist per hour, number of drug boxes dispensed per pharmacist per hour), patients’ medication-waiting time and dispensing errors were compared among four periods: before scanning implementation (June 2024), before management intervention (June 2025), the first phase after management (September 2025), and the second phase after management (December 2025), so as to evaluate the management effects.RESULTSThe traceability code scanning rates in the first phase and second phase after management were remarkably higher than those before management (P<0.05), and the scanning rate in the second phase after management was obviously higher than that in the first phase after management (P<0.05). Compared with the period before scanning implementation, the number of prescriptions dispensed per pharmacist per hour, the number of drug boxes dispensed per pharmacist per hour, decreased significantly and patients’ medication-waiting time prolonged markedly before management (P<0.05). In the second phase after management, both the number of prescriptions dispensed per pharmacist per hour and the number of drug boxes dispensed per pharmacist per hour increased significantly, and patients’ medication-waiting time shortened greatly in contrast with the status before management (P<0.05). Besides, no statistically significant differences were found in the number of drug boxes dispensed per pharmacist per hour and patients’ medication-waiting time between the second phase after management and the period before scanning implementation (P>0.05).After the implementation of traceability-code scanning management, dispensing errors presented a gradually declining trend in each stage.CONCLUSIONSThe fine management system for outpatient drug traceability codes constructed based on Pareto chart and ORTCC model can improve the traceability code scanning rate and pharmacists’ work efficiency of outpatient pharmacies, and reduce drug dispensing errors.
