目的:利用T>MIC计算模型评价不同T>MIC下亚胺培南的临床方案及疗效。方法:采用回顾性方法,选取2015年10月-2017年6月在中日友好医院住院期间行亚胺培南血药浓度检测的患者107例,记录其基本信息和用药情况,运用SPSS 19.0软件按T>MIC计算模型计算不同给药方案下不同患者亚胺培南的T>MIC,依据不同T>MIC分为≤40%组、>40%~70%组、>70%~100%组和>100%组,比较各组患者的亚胺培南用药持续时间、降钙素原(PCT)降低百分比。结果:纳入研究患者97例,其中≤40%组18例、>40%~70%组35例、>70%~100%组34例、>100%组10例。4组患者基线水平差异无统计学意义(P>0.05),其亚胺培南用药持续时间分别为(10.7±2.69)、(8.71±3.48)、(8.50±4.21)、(9.00±4.87) d,PCT降低百分比分别为(37.00±28.83)%、(53.43± 21.66)%、(58.67±16.53)%、(47.25±24.71)%。结论:临床使用亚胺培南进行抗感染治疗时调整治疗方案使其T>MIC在>70%~100%范围内能获得较好的抗菌效果。
OBJECTIVE:To evaluate clinical program and therapeutic efficacy of imipenem by using T>MIC calculation model under different T>MIC. METHODS: By retrospective method, 107 patients underwent blood concentration monitoring of imipenem were collected from China-Japan Friend Hospital during Oct. 2015-Jun. 2017. General information and drug use were recorded. T>MIC of imipenem in different patients receiving different programs was calculated according to T>MIC calculation model by using SPSS 19.0 software. According to different T>MIC, those patients were divided into ≤40% group, >40%-70% group, >70%-100% group and >100% group. Medication duration of imipenem use and the percentage of PCT decrease were compared among those groups. RESULTS: Totally 97 patients were included in the study. There were 18 cases in ≤40% group, 35 cases in >40%-70% group, 34 cases in >70%-100% group and 10 cases in >100% group. There was no statistical significance in baseline level of the body among 4 groups (P>0.05). The medication duration of imipenem use were (10.7±2.69),(8.71±3.48),(8.50±4.21) and (9.00±4.87) d, respectively. The percentage of PCT decrease were (37.00±28.83)%,(53.43±21.66)%,(58.67±16.53)% and (47.25±24.71)%, respectively. CONCLUSIONS: When imipenem is used for anti-infection treatment, that therapy program is adjusted to maintain T>MIC between >70%-100% may achieve good antibacterial efficacy.
