OBJECTIVE： To draw lessons from chronic disease management model in the overseas， and to promote chronic disease management in China. METHODS： Retrieved from PubMed， CNKI， Wanfang database， etc.， the literatures about chronic disease management model were summarized and analyzed. The improvement methods were put forward for domestic chronic disease management. RESULTS&CONCLUSIONS： The chronic disease model in the overseas mainly included chronic care model， chronic disease self-management model，transitional care model and British chronic disease management system model， etc. In China， chronic disease management model mainly included chronic disease information monitoring system model， chronic disease self-management model， community chronic disease health management and community chronic disease clinical pathway management model， etc. Referring oversea experience， Chronic disease management make certain progress and still sheald be improved from aspects of clinical information system， health system， patient self-management， community resource and policy support， etc.
