clinical pharmacists assisted clinical physicians to judge clinical efficacy of clopidogrel and develop individual dosing regimens of warfarin; predicted the dose of sodium valproate by using population pharmacokinetics; formulated individual medication guide by using blood concentration monitoring. Better clinical efficacy had been obtained and ADR reduced. CONCLUSIONS: Precision pharmaceutical care can effectively provide patients with optimal dosing regimens

predict initial dose and optimal therapeutic concentrations

improve efficacy and reduce ADR.
