OBJECTIVE: To explore the role of clinical pharmacists in medication errors (ME) recognition and prevention, and to formulate related ME prevention measures. METHODS: 106 cases of ME were collected from our hospital during 2013 to 2014, and analyzed statistically in respects of types, reasons, involved drugs and staff. Then some risk prevention approaches were proposed under the existing circumstances. RESULTS: Among 106 cases of ME, type error accounted for 47.17%; reasons for ME mainly were similar appearance and general name, accounting for 25.47% and 17.92%; involved staff mainly were primary pharmacists, involving 87 cases (82.08%). CONCLUSIONS: Practical measures for ME prevention included implementing system management, strengthening electronic and informatization management, establishing reasonable medication network platform, improving working environment and staff training, etc. Pharmacists can play role on ME prevention, which have the positive effects on improving safety consciousness in physicians, nurses and patients, reducing ME and promoting rational drug use.
